Prescribed Medication & Plan of Care

A reminder to parents that any prescribed medication (e.g., EpiPen, puffer, antibiotics) that a student is required to take during the school day, must be accompanied by "Form 1 - School Personnel to Administer" and/or "Form 2 - Self-Administration". Families are encouraged to visit their Health Care Provider prior to the start of school to ensure all forms are completed for the first day of school.

If your child has a prevalent medical condition (e.g., Anaphylaxis, Diabetes, Epilepsy, Asthma) and currently has a Plan of Care for the 2018-2019 school year, please complete the attached Annual Review of Medical Information Form to indicate if there are any changes in regards to your child’s Plan of Care / Administration of Prescribed Medication or if there are no changes for the 2019-2020 school year.


To help ensure a smooth transition back in September, please return the completed Annual Review of Medical Information before the end of June. Completed forms can be submitted to the office. Your attention to this matter is appreciated, as your child’s health and safety while at school is of the greatest importance to us.



Of note, all epipens and inhalers housed at school will be returned to student on the last day. If your child has additional medications at school, a parent/guardian is required to come pick it up from the Main Office – thank you.
